BEFORE YOUR VISIT

• Our new patient health history questionnaire MUST be completed two days PRIOR to the time/date of your appointment.

• If you have any changes - new insurance information, new pharmacy, new address etc. - please call (727) 807-5900 to update your information. 

• Complete the COVID screening questionnaire on the patient portal.

• Call and provide card information for any co-pays or any unpaid balances.

WHAT YOU CAN EXPECT AT YOUR VISIT

• Our goal is to eliminate the use of our waiting area, so pleas stay in your vehicle and call the office when you arrive (727) 807-5900.

• We ask that you do wear a mask to your visit.

• A clinical staff member wearing gloves and a mask will obtain your temperature.

• Our staff will take you directly to an exam room when we determine if you are ready.

• Curbside check-in and pre-screening

• Eliminating all unnecessary objects from our waiting area & clinic

• Lower volume of patients to allow for proper social distancing
